Trillium Services provides Community Living Services (SLS) in Duluth and the surrounding areas. A community living arrangement can be developed for a single individual or up to four individuals in a typical single family home environment.
All of the homes Trillium Services operates are located in the Duluth, Hermantown, and Proctor areas and are licensed by the Minnesota Department of Human Services and St. Louis County.
We work with all individuals to determine their unique needs and use Person Centered tools to assist them in fulfilling them. In a group living setting these goals may look like money management, meal planning and preparation, personal care, mobility, and home maintenance to name a few. Our goal is for individuals to develop as much autonomy as possible in their daily life.
As their skills grow, individuals may move into our Transitional Planning and Connections programs if that is where their path leads..
We provide flexibility within the services delivered as well as the level of support provided to each individual. The person and their team will determine goals and objectives that they want to work toward and develop a plan to work to achieve them. We will facilitate the identification of what personal success looks like to them and help them strive toward it.
Daytime supports are determined by individual needs and all of our homes are staffed during overnight hours. We also offer single level accessible homes for those that may need an accessible home.

Trillium’s Connections program includes Home Supports with training and Semi-Independent Living Services (SILS), both of which are provided to adults who live in their own homes. These services are designed for people who live indpendently but would like assistance in certain areas to maintain their independence to live their best life.
This service provides support to individuals living in their own homes. Services are provided in their home and or in the community. We primarily assist individuals in this service with the continued development and maintenance of daily living skills. These skills may include but are not limited to:
The Connections Program assists individuals with transitioning from their family home or group living into their own apartment or house. This service assists with:
Family Services supports individuals and their families inside and outside the family's home. We provide In-Home Support Services (IHS). Respite Care services and various other customized person-centered services to support people and their families.
Trillium’s Family Services program provides in-home teaching and training along with recreational and social activities for individuals with a disability who live with their family or a caregiver.
In-home supports are a 1:1 service focused on empowering the individual to build skills and celebrate their success while doing so. Through IHS services, we build on the individual’s capacity each and every day.
These supports may range from community activities to teaching and training. Some examples of services include: social events, (concerts, hiking or walking in our local parks, going to the aquarium, museums etc.) cooking, money management, health and fitness, hygiene skills, medication management, safety skills at home and in the community, and relationship building.
Trillium provides respite care services to families and caregivers as a temporary break from their day-to-day lives. Respite care can look ing the following:
Respite care can range from a few hours on a weekend to extend periods of time depending on the family’s needs and circumstances.
Respite care is intended to give families a well-deserved “rest” from their day-to-day lives. Trillium is happy to provide a trained staff to help these families take the time they need to recharge, whether it is only for a few hours or extended periods of time.
We offer Designated Coordinator and Designated Manager consulting services to family foster care providers and smaller corporate providers. These services offer guidance, support, and best practices as required in MN Statute 245D. This service assists providers in maintaining compliance with 245D Standards while offering quality person-centered services.
Trillium Works! provides support for individuals who are seeking competitive employment or want to work on personally enriching life skills in their communities.
Employment Exploration helps a person gain a better understanding of employment opportunities in their community. They help to strengthen a person’s knowledge, interests, and preferences so they can make informed decisions.
Employment Development is the process of finding competitive employment that matches a person’s experience, skills, and interests.
Employment Support is the assistance a person needs to maintain competitive employment.
Individualized, community-based training and support services that help a person develop and maintain essential and personally enriching life skills so they can access and participate in activities they prefer in their community.
