We try to match people to the staff that will work with them by personality and interests that they have in common. Our services really come down to relationships and we have found that people have greater success when they work with people that they really like spending time with.
The answer to this question is different for everyone. What we can say with confidence is that everyone is better off working. We can assist each person in figuring out what would be best for them, so that they can set good goals for themselves.
Individuals in our program receive as much support as they need to accomplish their employment goals. Typically, this means getting a job and completing job duties to the satisfaction of the employer. The amount of support needed varies from person to person depending on their abilities. Support could be as minimal as occasional check-ins or as much as having a job coach that is always present and available to provide verbal and physical assistance as needed. In any case, the objective is for you to be successful in your employment goals and to strive towards greater independence.
The simple answer to this question is as long as is needed for you to be successful. The length of time a person receives supports can vary greatly from one person to the next. Some people only need supports as minimal as learning how to be successful in a job search or finding job openings. Others may need some level of support for their whole working lives. Many people land somewhere in between.
Trillium Works! can provide support throughout the entire job search process including but not limited to employment exploration and planning, developing a resume and other job search tools, finding employment, and working successfully.
The easiest way to begin services is to have your VRS/SSB Counselor or County Case Manger make a referral for services. The next step is to set up an informational meeting to make sure we are a good match for your goals. If we are a good match and you decide you want to work with us, we would then schedule an admission/intake meeting to begin services.
Trillium Works! can work with all individuals that qualify for a waiver through their counties including the Developmental Disability (DD), Community Access for Disability Inclusion (CADI) or Brain Injury (BI) Waivers. In these instances, funding comes from your waiver and is allocated for your support by your County Case Manager. We also do extensive work for individuals who are working with Vocational Rehabilitation Services (VRS) and State Services for the Blind (SSB). Services received through VRS and SSB are funded by the state of Minnesota.
Respite care is something Trillium offers to the families of those we support to give the family some relief and take a much-needed break that we all deserve from time to time! During respite care, we spend time with the service recipient for extended periods of time, assuring their needs are met, they are safe, well-fed, and having some fun. Respite care can be provided in your family home or at Trillium’s respite property.

The person served only pays for their own expenses. Trillium Services will provide transportation and pay for our staff’s expenses. Trillium staff also work hard to find free and low-cost activities in the community for those who may not be able to afford activities.
That depends on what you are interested in. The person served can give input on things they would like to do! You and your staff will work on Personal Outcomes which can involve recreational activities, exploring the community, learning to cook and menu plan, learning how to budget, working on personal care and independent living skills, and much more! Just ask your Trillium staff and we will figure out a way to make it happen!
That depends on whatever you choose and can vary from week to week. Some persons served have shifts every day and others only have one shift a week. A shift can last whatever you and your case manager choose, but Trillium has a minimum requirement of 3 hours. Respite care shifts can be 24 hours, all day, and overnight.
At Trillium, we try to be as flexible as possible! Once you choose a schedule, we just need a little time to make any changes you request. Requests for schedule changes with Trillium staff always should go through your Program Supervisor, and they will do their best to accommodate it.

Once you have decided on a schedule, your Connections team will send you a monthly schedule of the dates and times of your shifts and also who will be supporting you on those days. If something should come up where you might need to cancel a shift, need your staff to start a little earlier or stay a little later, with enough notice, that request can be easily accommodated.
Your Connections services take place not only in your home or apartment, but also in your community. Connections staff offer support in helping individuals gain and maintain important daily living skills such as banking, budgeting, and bill-paying, but also help support people in their community with grocery shopping, errands and recreational/leisure activities.

Yes, there are many ways that we can support your health and safety living on your own. We have the ability to utilize remote technology that is paid for through your Wavier. We also have a 24-hour Emergency Phone that you can call if something out of the ordinary should arise.
You do! At Trillium, we do our best to find the best staff match. When we interview a person, you’re a big part of the process. We give you the opportunity to interview potential employees through our Meet and Greet process. We take your feedback seriously and want to find the best fit for you!
The number of hours is decided by you and your team. Your Case Manager will discuss your level of needs and determine the number of hours per week that will support you the most.
At Trillium, we have our Transition Services available to anyone who is interested in living on their own. We can help with researching housing, determining the cost and a budget. From there, our Transition Services can assist you in securing your own apartment and also moving in!
We encourage the individuals we support to be as independent as possible and to take ownership over their home. We will cook and clean alongside them and encourage everyone to do their part.
We can do a lot! We work on things like budgeting and cooking. But we also make plans with friends, go shopping, plan and go on trips out of town, and find fun events around town to attend!
All of our staff get trained to administer medications. If a service recipient wants to administer their own, we can work with them and their doctor to do so.
Whoever you want! We can schedule appointments, bring individuals to appointments, or send paperwork with you if you choose to go yourself.
We do! If you would like to do so yourself, you absolutely can, otherwise we will make sure proof of income is sent in.
Room and board is a fixed number given to us. It is paid for between social security, earned income (paychecks), and Group Residential Housing (GRH).
We provide a bed, nightstand and dresser. We also provide bedding, pillows and towels- we’ll take you shopping so you can pick them out! Trillium writes a check for groceries to be purchased for the house, and Trillium covers utilities, including Internet and a house phone. You are responsible for your own clothing and personal needs items such as shampoo and toothpaste.
